Using our collaborative research, as well as on-the-ground experience, the AzPRC is developing a series of infographics to share best practices on how to integrate Community Health Workers (CHWs) into clinical practice. Our target audience is healthcare providers, administrators, and CHW supervisors. The infographics are designed to give the reader a quick snapshot of the topic and highlight additional resources. Topics to date include CHW 101, CHW Hiring, and How to Document CHW Work in the EMR. More infographics will be posted as they are created. Thank you to our CAB members for their invaluable input in this process!
