Arizona CHW Census and Organizational Inventory
Purpose:
This spreadsheet has been created to:
- Get an inventory of organizations that hire Community Health Workers (CHWs) in Arizona.
- Attempt to account for as many Community Health Workers (CHWs) as possible.
How CHWs Were Confirmed:
CHW counts were obtained by:
- Contacting CHW organization members (CHW program lead or worker) providing a count of employee and volunteer CHWs.
- Reviewing AzCHOW MAP Survey for organization responses.
Tab Organization:
Each tab contains confirmed organizations and a count for:
- Counties
- Community Health Representatives
- Cross-State (Organizations working in multiple counties or non-county specific)
Utilization:
This spreadsheet can be used for:
- Networking with local CHW organizations.
- Determining gaps in counties.
Disclaimers:
- Due to the nature of grants/program funding, CHW counts will change and vary.
- Contacts will change due to transitioning/removal of position in the organization.
- These changes may lead to missing or out-of-date information.
